Medical Aid Tax Credits Explained (South Africa)

Medical costs are one of the most commonly under-claimed items on South African tax returns — partly because the rules come in two parts that people confuse. Here's how the medical tax benefits actually work.

1. The medical scheme fees tax credit

If you pay contributions to a registered medical aid, SARS gives you a medical scheme fees tax credit — a fixed monthly amount for you as the main member, another for your first dependant, and a smaller amount for each additional dependant. It's a credit, meaning it comes straight off the tax you owe, not off your taxable income — so it's valuable regardless of your tax bracket.

You don't have to do anything clever to qualify: if you're the one paying the contributions and you're a member of a registered scheme, the credit applies. The exact monthly amounts are set by SARS each tax year, so check the current figures at sars.gov.za.

2. The additional medical expenses tax credit

The second part covers out-of-pocket medical costs your scheme didn't cover, plus contributions above a certain threshold. This includes things like:
